Sourcewise has been proudly serving Santa Clara County since 1973. We strive to educate, prepare, support and advocate for all adults in Santa Clara County. Our focus on helping seniors and caregivers continues as we expand our reach to adults with disabilities and all adults in Santa Clara County.

Our Community Resource Specialists provide personalized referrals to community-based services over the phone, by mail, and via e-mail. Through regular visits to senior centers, adult residential complexes, medical facilities, and other venues throughout the county, we educate individuals about our services and how they may benefit you and your loved ones – including in-depth Medicare presentations.

Sourcewise also provides you access to the Health Insurance Counseling & Advocacy Program (HICAP). This crucial service offers unbiased counseling services for those who have questions and need support navigating their Medicare options.

Through a home-delivered meals program: Meals on Wheels, homebound adults over 60 are provided a nutritiously-balanced diet. These individuals are otherwise unable to shop or cook for themselves and have no one to do so for them.

More active seniors are able to get back to work with our Senior Employment Services. After acceptance into this program, adults 55 and older (with low income) have the opportunity to sharpen skills and get back into the work force.

Care Management at Sourcewise provides support for seniors and caregivers who require a more personalized care plan. The Multipurpose Senior Service Program is available to help those who are 65 and older and have Medi-Cal with no share-of-cost. For the individual caring for a family member or loved one, the Family Caregiver Support Program offers respite for the caregiver.

Our Public Authority Services works with In-Home Supportive Services (IHSS) to build a network of caregivers who are trained and screened to help IHSS seniors or persons with disabilities living at home who need assistance with daily life activities.
