				<div class="et_pb_text_inner"><p><img fetchpriority="high" decoding="async" src="https://mysourcewise.com/wp-content/uploads/2023/11/infographic_Fall2023_web.jpg" width="300" height="182" alt="" class="wp-image-10442 alignright size-full" />Since the days when we were known as the Council on Aging Silicon Valley, Sourcewise has focused on serving Santa Clara County (SCC) with consistency and a constant drive to develop new and innovative solutions.</p>
<p>When Sourcewise doors opened in 1973, we only offered Senior Employment Services and Meals on Wheels. Fast forward 50 years later, we have added seven other programs, and a multitude of direct services, and opened an additional office in Morgan Hill. A few notable program/service additions include our Digital Inclusion, CalFresh Application Assistance, and Community Supports.</p>
<p>Beyond our direct services, we work with a network of grantees to broaden our impact in the community by providing funding for services that are critical to the lives of seniors and family caregivers throughout Santa Clara County. Among our grantees are organizations like Senior Adults Legal Assistance, Live Oak Adult Day Care, Family Caregiver Alliance, and more.</p>
<p>To ensure we are listening to all members of the SCC, we work closely with our Advisory Council. The Sourcewise Advisory Council provides valuable advice and insights; advocates on behalf of the most vulnerable; and, on occasion, informs the community about what resources and services are available through Sourcewise.</p>
<p>As we look forward to the future, we are always looking for ways to improve our current programs while also investigating new services that can better improve the lives of all persons with disabilities (aged 18 or older), seniors, and family caregivers in Santa Clara County. 				<div class="et_pb_text_inner"><p><img loading="lazy" decoding="async" src="https://mysourcewise.com/wp-content/uploads/2022/08/shutterstock_77179327-1-300x200.jpg" width="300" height="200" alt="" class="wp-image-8342 alignright size-medium" />The Medicare Annual Enrollment Period (AEP) is open now through December 7, 2023. The Sourcewise Health Insurance Counseling &amp; Advocacy Program (HICAP) offers counseling by appointment over the phone and by video conference. Face-to-face appointments are also now available at Sourcewise offices (on a limited schedule) and at select community centers and libraries.</p>
<p>Each year, drug and managed health plans may change premiums, benefits, and out-of-pocket costs (like deductibles and copayments/coinsurance) for the upcoming 2024 year as described in your September “Annual Notice of Change”.</p>
<p>The AEP allows existing Medicare recipients to look at their previous Medicare selections and adapt as needed:</p>
<ul>
<li>Change, enroll in, or drop a Medicare Part D prescription drug plan (PDP).</li>
<li>Enroll in or change a Medicare Advantage (MA) Part C HMO or PPO plan for its extra benefits and managed network.</li>
<li>Leave the managed Medicare system of MA Part C and instead use Original Medicare, a PDP, and Medigap for greater physician, provider, or facility flexibility.</li>
</ul>
<p>HICAP counselors can help you review changes, identify options, and plan your Medicare to meet your needs for 2024. HICAP will host AEP presentations called, “Medicare’s 2023 Annual Enrollment Period and Updates for 2024” via Zoom and at various community organizations throughout the county to keep you informed about the latest plan options and changes.</p>
<p>To prepare for an appointment, create a <a href="https://www.medicare.gov/" target="_blank" rel="noopener">Medicare.gov</a> account, especially if you take several medications. You can then view your Medicare history, including a list of recently filled prescriptions. HICAP Counselors can more quickly help you find the lowest cost drug plan on the Medicare.gov Plan Finder.</p>
<p>Save money on Medicare Part D drug costs through the Low-Income Subsidy program (also called “Extra Help”). To qualify for partial Extra Help, monthly income must be less than $1,823 for singles and $2,466 for couples. Or apply for Medi-Cal, which includes Extra Help for Medicare recipients and has effectively similar income limits.</p>
<p>Sourcewise HICAP counseling is free, unbiased, and available to Medicare- eligible Santa Clara County residents. 				<div class="et_pb_text_inner"><p><img loading="lazy" decoding="async" src="https://mysourcewise.com/wp-content/uploads/2023/11/iStock-1323185914-300x200.jpg" width="300" height="200" alt="Caregiver comforting senior" class="wp-image-10457 alignright size-medium" /><a href="https://svilc.org/" target="_blank" rel="noopener">Silicon Valley Independent Living Center</a> (SVILC) is a 501(c)(3) nonprofit that provides individuals people with all types of disabilities, including seniors with disabling conditions residing in Santa Clara County (SCC). All services are community-based and free to anyone seeking services to help further their independence in SCC. SVILC advocates for policies that ensure equal access and opportunity for all.</p>
<p>SVILC is one of twelve Device Lending and Demonstration Centers (DLDC) in California. Some items that can be available to borrow include wheelchair ramps, assistive listening devices, smart speakers and computer aids, accessible kitchenware and eating utensils, adaptive gardening tools, and voice options communication tablets for those with speech disabilities. SVILC also accepts donations of gently used and new durable medical mobility equipment, such as wheelchairs, scooters, and rollator walkers.</p>
<p>SVILC offers their new and improved Caregiver Registry. Their Personal Assistant Services Department locates caregivers that are actively seeking work for private pay or are hired through In-Home Support Services (IHSS). SVILC uses a Quick Match database where clients are able to select their needs, and the database creates unique lists tailored to each individual.</p>
<p>The Information and referral Coordinator at SVILC maintains updated information on community-wide services, programs, and seasonal events happening in SCC. Examples of information they can provide to clients include food pantries and clothing closets, locating benefits enrollment offices, utility payment assistance, and more.</p>
<p>Annually, SVILC directly serves 1,100 individuals with disabilities and provides Information &amp; Referral and Technical Assistance to another 4,000 residents and businesses in SCC.</p>

Centers for Disease Control and Prevention (CDC) emphasizes the importance of good lighting; installing bright lights, especially in hallways and staircases can help to enhance visibility and prevent missteps.</li>
<li><strong>Regular Exercise:</strong> Engaging in regular physical activity strengthens muscles and improves balance. Harvard Health advocates for exercises like walking, Tai Chi, and yoga. These activities can enhance flexibility and stability</li>
<li><strong>Reviewing Medications:</strong> Some medications can cause dizziness or imbalance. Mayo Clinic suggests seniors should have their medications reviewed regularly by healthcare professionals. Adjustments or changes can be made to prevent potential side effects leading to falls.</li>
<li><strong>Using Assistive Devices:</strong> CDC recommends using walking aids like canes or walkers if needed. Properly fitted and regularly maintained assistive devices can provide essential support, especially for those with mobility challenges.</li>
<li><strong>Vision and Hearing Checks:</strong> Impaired vision or hearing can increase the risk of falls. Harvard Health advises regular eye and ear check-ups to ensure optimal sensory functions, reducing the likelihood of accidents. </li>
</ul>
<p>By implementing these tips and accessing specialized programs, older adults can enjoy a safer and more active lifestyle, free from the fear of falls, while continuing to prioritize their independence and well-being.  </p>
