South County Resident Case Management
Everyone’s needs are different but everyone needs peace of mind.
Sourcewise Case Managers will work with you and your loved ones to assess your situation and develop an individualized plan of care that fits your social, physical, and economic needs. We connect seniors, persons with disabilities, their families, and caregivers to support services in our community: Morgan Hill, San Martin, and Gilroy.
To be eligible for South County Resident Case Management, you must meet the following criteria:
- Be a resident of Morgan Hill, San Martin, or Gilroy
- Be 60 years or older, or be an adult (18 years or older) with a disability in need of care coordination
- Be incapable to access services without assistance
- Be willing and able to accept case management

Our comprehensive case management services are provided by a team of professional Case Managers. Contact us by calling (408) 762-7362 if you or someone you know is needing assistance. Our Community Resource Specialists will collect all information needed and will connect you to a Case Manager.
The goal of our case management program is to enable you to remain safely at home for as long as possible, while maintaining optimal health and wellness.
Our Case Managers may connect you to programs & services offered at Sourcewise including: Meals on Wheels, Health Insurance Counseling & Advocacy Program, Caregivers Network, or Public Authority Services depending on your unique needs, and more. We can also connect you with external services and resources in Santa Clara County.
