Community Supports

Nursing Home Diversion/Transition Program

The goal of our program is to transition individuals from skilled nursing facilities back into a community setting. Community Supports offer new services provided by Medi-Cal managed care plans as cost-effective alternatives to traditional medical settings. The Sourcewise Community Supports program launched in January 2023.

How does it work?

  • The Community Supports team will help with discharge planning and transition individuals to an appropriate community setting. This can be a Residential Board and Care Facility (RCFE) placement or return to home.
  • Community Supports provides supplemental funds for care costs to maintain a supportive environment.
  • Wrap-around Care Management services – after placement, a Community Supports team member will check in monthly to assist members with any questions, concerns and ensure proper care is delivered.

Who We Serve

To be eligible for the Community Supports Program at Sourcewise, you must meet the following criteria:

  • Be 18 years of age or older
  • Receive Medi-Cal with Anthem Blue Cross in Santa Clara County
  • Need 24/7 Assisted Living Care
