Older Adults Home Modification Program

Older American Home Modification Program (OAHMP) will assist older adults and individuals with disabilities avoid injuries and enable them to continue to live safely in their homes. The program seeks to enable low-income elderly persons to remain in their homes through low-cost, low barrier, high impact home modifications to reduce older adults’ risk of falling, improve general safety, increase accessibility, and to improve their functional abilities in their home.

This will enable older adults to remain in their homes, that is, to “age in place,” rather than move to nursing homes or other assisted care facilities.

Eligibility

To be eligible for OAHMP, you must meet the following criteria:

  • Be 62 years of age or older or an adult with disabilities
  • Live in Santa Clara County
  • Live at home
  • Qualify for Medi-Cal or similar program
  • Provide verification of income
  • Be a homeowner on title
  • Provide proof of title

Contact Sourcewise if you or someone you know is at risk of falling. Our Community Resource Specialists will ask for all information that is needed. The call will then be followed up to do a fall assessment and then a plan of action on assisting on what home modifications will be needed to prevent falls. 

Once eligibility is determined by the Community Connections Advisor, the program will be reviewed in detail and the client will be contacted by the occupational therapist company called Periscope to conduct home safety assessments and provide their recommendations. All recommendation provided by Periscope will be done by Home Safety Services. Modifications are limited to cost.
