Let’s Get Started
Contact a Community Resource Specialist today at (408) 762-7362 to be screened for eligibility, and then be connected to the transportation program directly to schedule your first ride. You must provide your home address and the time you need to be picked up or dropped off. Please add in about 30 minutes of buffer time in case of traffic delays.
A driver will pick you up from your residence in South County, wait in the car for you while you shop, and drive you back home. A caregiver may accompany you if you need assistance. There is a limit of two passengers per ride.
To prepare for your first ride, make sure you have personal identification to show the driver and any belongings you may need for your travels such as a medical card for a doctor’s appointment or your wallet for the grocery store.
Existing Riders
If you are currently enrolled in the program and are expecting a ride, you will be notified by telephone of any emergency updates or delays on for your expected pick-up.
If you need to cancel a scheduled ride with one of our drivers, please do so at least 48 hours in advance of your scheduled service. Contact a Community Resource Specialist by phone at (408) 762-7362 or email southcounty@mysourcewise.com to let us know you need to cancel. It is your responsibility to reschedule the ride, if needed.
All of our drivers are experienced and prepared to drive you safely to your destination and it is not guaranteed that program participants will be paired with the same driver each time.
