We empower individuals by providing access to information, allowing for personal choices, and continued independence. We strive to create a community-based system of care that crosses city boundaries, income levels, geography, and special interests.

Our Impact
Sourcewise delivers a significant Community Impact in Santa Clara County. Since 1973, Sourcewise has been a pivotal icon in the support of seniors, their families, and caregivers through the delivery of programs and services to a rapidly growing senior population. The Sourcewise Area Plan—the blueprint for senior needs—guides our strategic planning, objectives, and goals to better support all adults, caregivers and persons with disabilities.
The California Department on Aging reviews and approves the Area Plans for all California Area Agencies on Aging, thereby releasing funds to allow these organizations to implement the outlined services and support of these plans.
