Supporting Healthy Lifestyles in Santa Clara County
The Sourcewise SNAP-Ed program is dedicated to empowering older adults with disabilities in Santa Clara County to lead healthier lives. Our goal is to provide nutrition education and resources to individuals who are eligible for the Supplemental Nutrition Assistance Program (SNAP), focusing on promoting balanced diets, physical activity, and overall well-being.
Population We Serve
We serve low-income older adults and individuals with disabilities who qualify for SNAP benefits. Our services are available to those in Santa Clara County who are seeking to improve their nutritional habits, maintain a healthy lifestyle, and learn more about managing food resources effectively.
At Sourcewise, we understand that access to healthy food and nutrition knowledge is crucial for our community’s well-being. Our SNAP-Ed program offers the tools, education, and support needed to make informed choices about food, nutrition, and physical activity, leading to healthier lifestyles and improved quality of life.
What We Offer
