No Gaps Here! How Linkages Helps to Ensure Clients Receive Uninterrupted Support
Linkages serves as an alternative care and case management option for older adults or adults with functional impairment who may be ineligible for care management services available through Medi-Cal. The program authorizes and arranges for the purchase of and referrals to services. Linkages can help ensure uninterrupted care and case management and prevent gaps in receiving services, and ensures a sustained focus on equity, especially for low-income adults.
Ote M., a veteran, had been unhoused since 2020. He enrolled into ECM from May 2023 through April 2024 until he lost his Medi-Cal coverage due to an unexpected increase in his share-of-cost. He was able to transition into Linkages and with the collaborative efforts of his Linkages care manager, his daughter, and the Veterans Affairs (VA) social worker, was able to successfully apply for and be accepted into a new affordable housing development built for individuals experiencing homelessness. Since May 2024 Ote has continued to live in this housing complex and credits his care managers with being able to find permanent supportive housing.
For another Sourcewise client, 64-year-old Annie B.* had been participating in the Sourcewise Enhanced Care Management (ECM) program from November 2024 through April 2025 when they abruptly lost Medi-Cal coverage due an abruptly high share-of-cost. By being able to smoothly disenroll from ECM and transition into Linkages, she was able to avoid having a gap in care and immediately begin figuring out ways to reduce her share-of-cost. As Annie has aphasia, anxiety, and depression, retaining access to a care manager allowed her to continue receiving support in the form of in-person visits and calls to county resources, legal resources, and senior housing complexes. She will be moving into an affordable housing apartment that empowers her to feel economically, physically, and emotionally safer than before.
