Care Management for Older Adults in Santa Clara County

Sourcewise Care Management programs support older adults and adults who need additional resources and care needs.  The high level of skill and knowledge our diverse care managers hold allows us to provide appropriate services to clients from many cultures and languages. Our Care Management team will work with you to assess your situation and develop an individualized plan of care that fits your social, physical, and economic needs.

Care Management encompasses three programs with distinct services and eligibility:

Community Supports (CS)

Community Supports connects you to essential non-medical services that support your overall health—like meals, transportation, and home modifications—to help you remain safely at home.

Enhanced Care Management (ECM)

ECM offers more intensive, ongoing support for people with complex health needs. A dedicated care manager helps coordinate your care across different providers and services.

Multipurpose Senior Services Program (MSSP)

MSSP is designed for Medi-Cal eligible adults over 65 who are at risk of nursing home placement. It offers in-home care, social services, and case management to help you stay in your community.

Additional Support Through Sourcewise and Community Partners

Our care managers may connect you to other Sourcewise programs based on your needs, such as Meals on Wheels, the Health Insurance Counseling & Advocacy Program (HICAP), the Caregivers Network, or Public Authority Services.

Depending on your situation, we may also refer you to a range of local resources, including:

Homemaker Services

Help with light housekeeping, laundry, and meal preparation to keep your home safe and comfortable.

Home Delivered Meals

Nutritious meals brought to your door if you’re unable to shop or cook on your own.

Home Safety Equipment

Access to items like grab bars, ramps, or medical alert systems to reduce the risk of falls and injuries.

Legal Services

Assistance with issues like powers of attorney, housing, elder rights, and other civil legal matters.

Transportation

Help getting to medical appointments, the grocery store, or other essential errands.

Day Care/Socialization

Structured programs that offer supervised care, social interaction, and activities during the day.

Money Management

Support with paying bills, managing income, and budgeting for daily expenses.

Respite

Short-term relief for caregivers so they can take a break while their loved one receives care.
